Matia Institute
The purpose of Matia Institute is to generate and transfer knowledge to contribute to the welfare of people and their environment throughout the aging process, through applied research from a people-centered approach.
The Matia Institute, together with Matia works on innovative projects to promote autonomy in daily life and the dignity of the elderly, with the ultimate goal of achieving their well-being.
Matia Institute is the Matia unit dedicated to the generation of knowledge and, therefore, the vehicle through which it makes its strategic vocation of generating applicable knowledge and promoting its transfer a reality, specializing in and promoting the application of its own knowledge. This is a hallmark of identity that constitutes a differential value of Matia compared to other institutions specialized in the field of aging.

Family Carers Ireland
Family Carers Ireland is the national charity supporting the 500,000+ family and young carers across the country who care for loved ones such as children or adults with additional needs, physical or intellectual disabilities, frail older people, those with palliative care needs or those living with chronic illnesses, mental health challenges or addiction.
Caring for a loved one can be rewarding, but it can also be difficult, frightening and isolating. It shouldn’t be. We want carers to know they have the support they need to care safely, to make it less of a battle and to know they are not alone.
TheFamily Carer Support Project purpose is to address a systematic approach to assess and respond to the support needs of families caring for an older person.
The objectives of the project are to secure referrals from the Health Service Executive and other community organizations, and to help family carers to build their coping skills through online supports.

St. Brendan's Village
St. Brendan’s Village Community Project is located in Mulranny, Co. Mayo, a small rural village in the west of Ireland, where a number of different services and community focal points have evolved beside housing units for older people. It is a not-for-profit registered charity run by the community for the community.
It provides total care through a continuum of support with people supported at home by day care, as well as meals-on-wheels, and extending to sheltered housing, and if required to high support care, which includes long term, respite and convalescent care, as well as palliative care. In this way people are living as much as possible, to their maximum ability, in their own area where everything is familiar to them, in an intergenerational setting.
It has stood the test of time for almost three decades, and is still the biggest local employer.
It makes perfect sense that older people who need support including long-term care should be facilitated to stay in their own community to receive that care.

Caregiver Bergamo
Caregiver Bergamo - Accanto a chi si predne cura is a project promoted by ATS Bergamo, in cooperation with the three local health authorities (Aziende Sociosanitarie Territoriali), the Collegial Body of Mayors (Collegio dei Sindaci), the 14 Ambiti Territoriali Sociali and numerous local authorities. Launched in 2023, the project aims to provide concrete support to informal carers, creating a solid network between institutions, health and social care services, volunteers and citizens, to build a more human, inclusive and shared care system.

ISIDORA
Isidora is an innovative digital platform developed by Cooperativa La Meridiana to support elderly people living at home and their caregivers. Conceived during the COVID-19 pandemic, it was designed to bridge the gap between seniors and essential health, social, and community services — transforming the home environment into a connected and engaging space.
Accessible via web or Android app, Isidora integrates four main modules that together create a comprehensive digital ecosystem:
It is part of La Meridiana’s long-standing commitment to building inclusive, sustainable, and human-centered solutions for elderly care, blending technology with social innovation.

Valakampiai Social Support Facility
Budgetary Institution Valakampiai Social Support Facility provides social care, supported living, development and support of social skills, and social workshop services for adults with disabilities in Vilnius. Its mission is to offer person-centred, high-quality services that empower people with disabilities, enhance their quality of life, and promote social inclusion. Long-term care supports individuals with complex disabilities in five homes for those who are fully dependent and need continuous support. The organisation partners with NGOs, public agencies, and sponsors to offer free education, workshops, events, publications, and staff. Valakampiai Social Support Facility aims to ensure modern, community-based, human rights-aligned long-term care that fosters independence, empowerment, and social inclusion.

GENCAT - Department of Health and Department of Social Rights and Inclusion - Integrated Social and Health Care

Salut+Social (Health+Social Project)
The Salut+Social (Health+Social Project), launched in Amposta in June 2014, is an innovative initiative that promotes integrated home-based care for individuals with complex health or social needs. Through close collaboration between the Catalan Health Institute (ICS - Institut català Salut) Terres de l'Ebre, and the municipal Basic Social Services (SSB), the project’s main goals are to ensure comprehensive care, improve professional collaboration, avoid duplication, and focus on the person and their support network. This model represents a paradigm shift toward integrated, person-centred care with a transformative approach to the care system

Solidaris - Federation of Home Care Centres (FCSD)
Solidaris, one of the most important mutual in Belgium, aims to promote fulfilment and improve living conditions for all.
As a counterweight to structures that seek to weaken social protection, the organisation offers more than just reimbursement to its members. It also aims to provide them with a wide range of services, supporting and guiding them on a daily basis, particularly in overcoming the difficulties they may face.
Solidaris thus sees itself as an innovative social entrepreneur, even an experimenter, working for the well-being of its fellow citizens. As part of Solidaris, the Federation of Home Care Centres (FCSD) comprises eight Home Care Centres (CSDs) and seven associated services. These services employ more than 3,000 workers who enable people who wish to do so to remain in their own homes for as long as possible, despite reduced independence due to age, illness, disability, accident, etc.
